Regarding Transition to approved pergolide

    One of the most important things I feel in transition to approved pergolide is getting a baseline on ACTH or ODST Day 0..we cannot assume a horse is already controlled on compounded drug when making the transition; can properly titrate dose then based just on response to the approved form from there on out...recommended recheck 30 days--also taking into account improvement in 1 or more clinical signs during that initial time. This is very important for both existing PPID cases and newly diagnosed cases starting drug for the first time. If a horse is transitioning to approved pergolide and no baseline is obtained on/approx Day 0, a point ACTH sample later on is difficult to interpret. For example, one doesn't know if the horse started with an ACTH of 1100 and is down to 80 (improved!), or if 80 is an indication that the dose should be increased...
