Clomipramine for Separation Anxiety

    Q:  I am curious as to whether there is ever an increase in the stress levels in the beginning of use of clomipramine for SA or would that be considered a contrary reaction to the meds?

    If a patient seems more agitated on ANY medication, I generally discontinue the medication.  In some medications group, such as the benzos, increasing the dose may actually help.  

    In some cases, there can be initial agitation that goes away after a few weeks of treatment.  However, I do not continue the medication in those cases.  Some of my colleagues do.
